The story follows Ageha Yoshina, a troubled high school student with a strong sense of justice who offers his services to solve his classmates' problems for money. His ordinary life is fractured the day he finds a mysterious "red calling card" abandoned in a public booth with the word Psyren printed on it. Shortly after, his childhood friend and classmate, Sakurako Amamiya, who also possessed one of these strange cards, disappears without a trace. Desperate to find her, Ageha follows clues linking him to a series of nationwide mass disappearances orchestrated by a secret society. Upon using the card, the young man is dragged against his will to a devastated dimension populated by grotesque monsters, where he is forced to participate in a deadly survival game. With his life in constant danger and awakening latent psychic abilities, Ageha must uncover the truth behind this organization if he wishes to see his home again.
